Ibogaine and its metabolite noribogaine have pharmacology that can affect cardiac electrical signaling. In particular, laboratory work has raised concern about blockade of the hERG potassium channel, a pathway associated with delayed cardiac repolarization and QT prolongation.
That concern is not an abstract technicality: the FDA’s discussion of QT-related abnormal rhythms illustrates why changes in repolarization are treated seriously in drug safety assessment.
